About AlēvCare

When you or a loved one are diagnosed with a life-limiting illness, every moment matters; especially the moments spent with family, friends and enjoying your favorite activities. At AlēvCare™ Hospice, our pain management and hospice care experts help to make the most of life’s precious moments. Through Hospice care, patients maintain dignity, self-respect, and personal control over their lives.

AlēvCare™ provides and pays for these services:

• All clinical staff services, including our physician Medical Director, RN Case Manager, Home Health Aide, Medical Social Worker and Chaplain. Volunteer services are also available

• All medications associated with the terminal diagnosis for symptom control and pain relief

• All needed medical equipment, such as oxygen, a hospital bed, wheelchair and more

• Respite care as needed, through paid placement of the patient in a long-term care facility for 5 days

• 24 hour emergency on-call services

• 13 months of bereavement care for family members

You will have an experienced team of physicians, registered nurses, home health aides, medical social workers, counselors, and trained volunteers available to help you and your family manage your illness.

Although it may be difficult to shift your focus from seeking a cure to choosing comfort care as the best alternative, numerous studies have shown that patients enrolled in hospice services actually live longer with better symptom control and pain management than non-hospice patients. The most common statement made by families who chose hospice for their loved one is, “We wish we had known about hospice sooner.”
