When to Consider Hospice Care
Hospice is specialized care for people living with a serious illness when the focus shifts from cure to comfort, connection, and quality of life.
Families often begin hospice when they notice:
- More frequent hospital stays or ER visits
- Increasing pain, shortness of breath, or other distressing symptoms
- Significant weight loss or increasing weakness
- Spending more time in bed or in a chair
- More help needed with bathing, dressing, walking, or eating
- Confusion, memory changes, or difficulty communicating
You do not have to wait until the very last days of life. In fact, starting hospice earlier often means better symptom control, fewer crises, and more meaningful days at home.

What Happens When You Call AlēvCare Hospice
You don’t need to have everything figured out before you call.
And you don’t have to wait for a doctor to bring up hospice.
Anyone can refer, including a family member, friend, neighbor, pastor, or clinician.
Step 1: Call or Submit a Referral
You can call us directly or submit a referral online. A member of our hospice team listens to your story, gathers basic information, and helps determine the next steps.
Step 2: Nurse Review & Eligibility Discussion
A hospice nurse reviews medical information, talks with you, and coordinates with the physician. We explain whether hospice is appropriate, what it would look like, and what choices you have.
Step 3: Admission & First Visits
If hospice is appropriate and you choose to move forward, we schedule an admission visit, deliver needed equipment and medications, and begin services. In many cases, we can admit within 24 hours.
Our goal is to make the first call as simple and calm as possible. There is no obligation, only information and support.
What to Expect in the First Few Days
Starting hospice brings more stability, not less.
In the first days, families can expect:
- A thorough assessment of symptoms, needs, and priorities
- Delivery of equipment such as a hospital bed, oxygen, or other items for safety and comfort
- Medication review and adjustments focused on comfort and quality of life
- Introductions to your care team, including the Nurse, Aide, Social Worker, Chaplain, and Child Life Specialist (if appropriate)
- Teaching and coaching so caregivers understand how to use medications, supplies, and equipment
- It can feel like a lot at first, but very quickly families often describe feeling more supported, less overwhelmed, and no longer alone.
Who Is Eligible for Hospice Care
Hospice eligibility follows Medicare guidelines, but every person is unique. Someone may be eligible if:
- They have a serious, life-limiting illness such as advanced heart, lung, kidney, or liver disease, cancer, dementia, Parkinson’s disease, ALS, or frailty with multiple chronic conditions
- A physician believes life expectancy may be about six months or less if the illness follows its usual course
- Curative treatments are no longer effective, appropriate, or desired
Hospice is a 100% Medicare-covered benefit, and most private insurances have similar coverage.

Miss Becky came into my life from a referral. What an absolute Godsend she is! Her crew were quick to get a hospital bed and supplies to my house and all of the people she sent were the very best at what they do. First it was a company called Horizon that came and set up the bed, oxygen and supplies. Mark at Horizon was professional and fast at delivery and pick up. They also had medications delivered to my door! Then it was the Case Worker, Tonya, and the RN Carlie that made decisions that were best for our situation. Two nurse angels from Mercy named Carol and Kristin took shifts to keep my darling husband comfortable while I grieved. There was also Nurse Celia who was so compassionate and patient with my confused husband!! Miss Becky had everything under control during a heartbreaking time for me and from start to finish the whole team was compassionate, professional and extremely competent. I recommend AlevCare Hospice very highly should your family be in the heartbreaking situation we were in. I call them the parade of angels that show up and manage the entire process for you. May 29, I received a text from the nurse who helped us Annie today to see how we were doing. We were surprised but also thankful. THis is not a company who forgets about you after your relative passes. Also thank yo for responding Mr. David. For replying to all our messages. All our thanks. First review --->>>Last month we used this hospice for my dad. From the very start when we called then when the nurse came out to explain hospice. I was personally very nervous because I had the wrong idea, that they just stay and give the person medication to sleep until they pass. The nurse was very informative and brought a calm presence to us. Most importantly she gave us things to do to help my dad feel comfortable and explained what would happen next and what to expect. I was worried my dad would dehydrate and suffer but learned that the body stops wanting food and water because it needs that to be peaceful. Within a few hours of the nurse coming and getting him on hospice we had a bed and oxygen and medication for pain and breathing and anxiety. He was not on hospice very long, just one day, but he was able to die peacefully. My mom and grandma were able to relax as much as they could knowing he was in good hands. We looked at one other hospice the hospital recommended but I am so glad we chose Alevcare. Thank you so much to the nurse that came out, and the person that gave him the bath. I don't remember their names but thank you. Also to the people who called after he died too. We truly felt your care even in that quick day he was on hospice. RIP dad.

My Dad has been gone a year now. As I reflect on the last 4 months of his life, after we elected to put him on Hospice Care and keep him at home, I am grateful for that decision, and especially for choosing AlevCare Hospice. The caregivers were amazing. Each one from his admitting nurse, to his care manager RN, who stopped in to check on him not only on her reg scheduled times but any time I asked her for an extra visit; his wonderful Social Worker, who quickly keyed in on his status as a WWII VET and engaged with him about his life as a missionary; to his lovely CNA who was able to get him cleaned up and kept us all sane knowing he was treated gently and respectfully. His Chaplain gave us, his children, and grandchildren caring, loving, help the last 3 days Dad was alive.I am an RN with 15 years of Hospice experience at 3 different companies, I truly can say AlevCare was far and away the best Hospice team I could have asked for. Once the decision was made to have hospice care for my husband we were referred to AlevCare. A case worker came out to tell us about AlevCare services. She was very caring and understanding and did not pressure us to make a decision but after all areas of hospice care were explained we chose AlevCare. From the minute we made our decision we were treated with the upmost care and compassion. The nurse assigned to us was Meagan. I have only high praise for her. She truly made us feel like family rather than just a patient. Destiny was his aide. My husband was a very private person and she made him feel at ease. The chaplain came to visit and had prayer with us. There were others that came and all were professional yet very tender in the care for my husband. When the time was near Meagan came and provided a calmness in our home. She respected our privacy and explained what to expect and allowed us to be with him as he peacefully transitioned to Heaven. Having a loved one on hospice is so hard, but Alēv Care makes it easier.They have a person for everything, including a child specialist to help my 11 yo navigate having grandma spend her final months in the comfort of our home. My child doesn’t usually open up well to adults but she took an instant liking to Kelly and looks forward to their visits. I love that we can continue this service after grandma passes so my daughter has someone besides her parents to guide her through her grief. I lost my grandma around this age and I could’ve used someone like Kelly to help me with my grief.They are flexible, which helps me make sure my mom is supported while I’m also about to make school pickup on time.Our nurse Sarah is responsive and very flexible. She listens to feedback on medications and adjusts accordingly so my mom is comfortable but not zombie-fied. When my mom ran a fever in the evening, Sarah made sure an on call nurse came to us quickly.They’re only here a few hours a week right now since my mom is not quite ready for a lot of care and the amount of weight they’ve lifted from my shoulders is so much.My mom loves Lupe. We love Lupe too, but my mom looks forward to Lupe coming to help her during the week.I definitely couldn’t do this without the respite care which Tanya coordinates with so much care and attention to anything my mom needs. I know she’s safe so I can enjoy small breaks with my family as we have been caring for my mom since shortly after she was diagnosed with her illness 5 years ago.Everyone we’ve dealt with has been incredibly supportive and you can tell this is more than a paycheck to them. And nothing feels rushed or like they’re just waiting for my mom to die. They’re committed to making her final months comfortable and happy, and they are happy to do that for as long as my mom needs.I couldn’t ask for a better support system ❤️

Alevcare Hospice made a very difficult time much more bearable. From the kind and compassionate nurses, to the ongoing help from the child-life specialist, Alevcare had everything handled for us. We got medical equipment, prescription meds, and emotional support for me and my children both before my husband passed away and since. The nurses, chaplains, and child-life specialist were all incredible and prayed for us and with us. I am especially grateful for Cori, Tonya, and Kelly. My kids hug Kelly when she comes to “play” and they tell her they miss their dad and talk about their emotions in a way few kids understand how to do at such a young age. AlevCare was such a blessing to our family. We didn’t really understand the difference between palliative care and hospice and didn’t think we were quite at the hospice stage yet with my GranGran. Shannon guided us through the process and explained everything. AlevCare took so much of the weight off of our shoulders. We were able to spend more time loving on our GranGran while Angela and AlevCare took care of her medical needs. We absolutely loved our nurse, Angela. She was an angel. Needing hospice care is a very painful realization. If you choose this agency you'll get excellent nurses. They are knowledgeable, capable, kind, patient, and empathetic. Our aide came daily (M-F). She not only took care of my husband by bathing, lotioning, & changing him, but did so gently and with compassion. She also taught me how to care for him and tricks and techniques to do so. My husband’s sister had a stroke and the other nursing home was not giving her the care she needed. And we knew she needed hospice care as her health was declining. There is no possible way I can express my gratitude for all the wonderful people at Alev Hospice Care. I could not have had the strength to be my mom’s caregiver the last week of her life if it was not for everyone at this amazing establishment. They gave me the guidance, confidence and support every single day throughout the process. As soon as we made the decision, I called the owner, David, on his cell phone late at night and before a holiday. He immediately answered the phone explained how his team helps care for your loved one and put me at ease. Before 11 PM, a nurse was at my home enrolled my mother, and medications were delivered immediately to keep her comfortable. This is not an easy decision to make, but everyone nurse, nurse’s aide, equipment delivery drivers showed such genuine compassion for my mother, myself, and my family. They were truly available 24/7 and wanted to make sure I felt supported and confident being my mother’s caregiver. This company has such high standards and everyone that works here loves their job and the company. I HIGHLY recommend ALEV to anyone going through this experience. I decided to share our experience with AlevCare. I know it can be hard to make a decision like this as it pertains to loved ones. My mom and I switched from another hospice company over to AlevCare. It was hard, because neither one of us was completely sure of what we were doing through this arduous process. We were not disappointed by our decision. AlevCare has given me a gift that will enable me to live the rest of my life without guilt and remorse. I have only a feeling of calm when I reflect on how AlevCare cared for my Mom in her final days.As exhaustion from sleep deprivation and mental fatigue set in to family care givers or single caregiver, decisions that should be obvious, become difficult to make. Making the decision of turning ones dying Mother or loved one, over to complete strangers while in this foggy state of mind is very very difficult. However, at some point, this must occur or there will soon be two patients as the family caregiver becomes the patient due to physical and emotional breakdown. Walking out the room where ones loved one is, to get what seems like a few hours of selfish sleep is difficult, actually far more than difficult. Leaving a dying loved one with a person who you met only a few minutes earlier, now in complete control of medication issuance and life and death service....is extremely difficult to say the least. Leaving the person who gave you life, who has made made late night sacrifices caring for you as an infant, to a stranger, seems selfish in order to get a few hours of sleep. But, this must be done.This is not my first experience with a hospice company as I have been through hospice three times with family members.Regarding AlevCare....there can be no better hospice company. From inception of the hospice decision to moms passing, the service was seamless, gentle, caring and professional. From the management team to the actual hospice nurses, helping my Mother pass was done by AlevCare as perfectly as hospice can be provided.When I met with the AlevCare management representative, with the lead nurse to establish the hospice plan and with our actual hospice nurse...well I was confident that I had made a good decision to select AlevCare. When the time came for me to actually leave Mom and get a few hours of sleep, I was calm as I left my sleeping Mom with the AlevCare hospice nurse sitting quietly beside her bed, noting times and types of medication issued. I went home got some much needed sleep. I met other AlevCare nurses over the next few days and each one brought the same professional, caring, service.When the last day came, AlevCare phoned to let me know that Mom had passed while I was out trying to keep my personal life going which also has to be done. Mom had passed after seeing myself and my brother daily and all her sisters in the previous few days. Her passing at that time did not surprise me as this is not uncommon for hospice patients elect to pass after loved ones have all been seen for the last time.I arrived at Mom's apartment at Legend Senior Living in Mansfield, a wonderful residence, only a few minutes after she passed but the AlevCare nurse on duty in her room was compassionate, informative and professional about what had just transpired and what had transpired in the morning hours before.I can sleep again now, because I know Mom is in heaven and had the best care possible. The folks from AlevCare Hospice we’re so kind and caring during the final weeks of my mother’s life. I can’t thank Angela enough for the compassion and friendship she showed toward my 92-year-old mother who suffered from dementia. All though the experience of the ultimate loss of a stick and or elderly love one, plays out on a roller coaster of emotions. I have to say that working with Shannon and the other wonderful people at Alevcare hospice during that time, was a god send. Even with the rolling blackouts freezing weather her group kept me informed, updated and no matter when I called they were there for me to answer questions and address any concerns, my family’s issues too and most of all they were there for my mother.Truly a great group of people to lean on during trying times, I feel that you can really count on them.

I couldn’t have managed my mother’s end of life care without AlēvCare. Thank you for everything you did to make her comfortable & feel loved! The minute I talked to David, the owner, I felt like I made the right decision. Rhonda, their social worker, helped me find a new home for mom. Then her nurses, Kathi & Angela, were exceptional. I also know there were nurses behind the scenes that were involved in helping mom. Plus their chaplain Art visited with mom often. I highly recommend AlēvCare Hospice! My family was new to the hospice experience, and our experience didn't last as long as I would have liked it to..... however, the nursing staff went above and beyond to help me understand what was happening and why it was happening so fast. We had some amazing nurses.... but the ones that really got us through this storm were Lisa (night nurse), Kat and Janet. Honestly... they were angels from above. They were kind, thoughtful, compassionate and just overall amazing! I will forever be grateful to them and this uncomfortable journey that they walked us through.

Recently, a family member was diagnosed with stage 4 cancer and he went downhill fast. He had no desire to enter a hospital and we had no clue what to do or where to find a reputable hospice service. Thankfully, we have a friend on the east coast that's a hospice nurse so we talked to her. After speaking with her she said he was definitely a candidate for hospice and she would do all the research to find a hospice service here in the Dallas Fort Worth area that could help us. She recommended AlevCare so we did a search and saw they only had 5 reviews at that time. Honestly speaking, I was highly hesitant to use you because of the low amount of reviews but after speaking with them on the phone and meeting the nurses it was an absolute no brainer decision.I won't mention the names but the nurses, the nurses aide, and last but not least, the social worker, they each played an integral part in taking care of the patient and the family. They made us feel like we are part of their family.When we texted ---- they responded quickly. When we called, they either answered or got back to us quickly. It didn't matter the time of day.The patient had no desire to extend his life with treatment and he made that obvious to the staff. He was very opinionated on the path of treatment but he absolutely adored each of the staff members responsible for his care.Everyone did an amazing job but we would really like to give accolades to the nurses aide that took especially great care of him and helped in the bedroom doing chores she was not asked or expected to do. She is an angel.After the patient's death, the owner called to check on us and asked how we was doing and if we we had any recommendations to make the hospice service better. Absolutely not! It's obvious you and your employees are a blessing to everyone that uses your service.On behalf of all our family we thank you from the bottom of our heart for taking such great care of us during this sad time of our lives. In the midst of the pandemic, my 92 year old mom suffered a stroke and we chose to have her come to our home with support from hospice. At the recommendation of a good friend, we chose AlevCare. They were professional, deeply caring, attentive to mom's needs, knowledgeable and responsive to our concerns and questions. Mom loved seeing the CNAs (Whitney and Jacklyn) each day and looked forward to visits from her RN, Angela. After 3 months, when mom suffered another stroke, they responded quickly and compassionately. She was on continuous care for less than 2 days and they were here to support us as mom took her last breath.I only know by hearsay what friends have experienced with other hospice agencies, but I would say AlevCare did a wonderful job in every way: before, during and after our time with mom. Thank you.
