Hospice Care in Parker County

Parker County has a strong sense of community, proud ranching and farming roots, and the growing towns around Weatherford and Aledo. People here value relationships, reliability, and people who do what they say they will do. When families face serious illness and begin considering hospice care, they want those same values reflected in the care they choose, not a distant company that feels disconnected from their life.

AlēvCare Hospice is a locally owned hospice, honored to serve patients and families throughout Parker County, with clinically excellent care delivered with a hometown feel.

Working with Parker County Hospitals and Facilities

Many Parker County families receive care across multiple settings, such as local clinics and physician offices, hospitals in Fort Worth or surrounding cities, or in assisted living or skilled nursing communities in the county.

Our team works to keep care coordinated by:

  • Communicating with your loved one’s physicians when appropriate
  • Coordinating with discharge planners and case managers
  • Staying in close contact with facility staff if your loved one lives in a community setting
  • Helping families understand what to expect as care transitions from hospital to home or facility

Our goal is to reduce confusion and help everyone stay on the same page, no matter where care begins.

Common Questions from Parker County Families

  • “Is it too soon to call, or have we waited too long?”
If you are asking this question, it is usually time to at least explore hospice. We can review your loved one’s condition, talk through what you are seeing, and help you understand whether hospice is appropriate now.
  • “Can you help if our doctors are outside Parker County?”
Yes. Many patients in Parker County see physicians in Fort Worth or other nearby cities. Our hospice Medical Director and nurses coordinate with those providers so the plan of care stays aligned.
  • “Will you still help if my loved one is in a facility?”
Yes. Hospice can be added in assisted living, residential care homes, and skilled nursing facilities to provide an extra layer of support.

The WHOLE STAFF was professional, compassionate, informative, patient, and responsive. Their team made the whole process of mom's passing less stressful. They were caring and comforting. My sister and I are very grateful for the service of AlevCare Hospice of Mansfield, Texas.

My mother was cared for by AlevCare during the last few months of her life. From the administration, volunteers, chaplains, aids and especially the nurses the care she received was top notch. My sister and I feel that their compassionate and thoughtful care greatly improved the quality of her life. Additionally, they communicated with us frequently and we knew what to expect. Since Mom passed (gently and painlessly)they have been very supportive for our family.

I highly recommend AlevCare.

Miss Becky came into my life from a referral. What an absolute Godsend she is! Her crew were quick to get a hospital bed and supplies to my house and all of the people she sent were the very best at what they do. First it was a company called Horizon that came and set up the bed, oxygen and supplies. Mark at Horizon was professional and fast at delivery and pick up. They also had medications delivered to my door! Then it was the Case Worker, Tonya, and the RN Carlie that made decisions that were best for our situation. Two nurse angels from Mercy named Carol and Kristin took shifts to keep my darling husband comfortable while I grieved. There was also Nurse Celia who was so compassionate and patient with my confused husband!! Miss Becky had everything under control during a heartbreaking time for me and from start to finish the whole team was compassionate, professional and extremely competent. I recommend AlevCare Hospice very highly should your family be in the heartbreaking situation we were in. I call them the parade of angels that show up and manage the entire process for you. Thank you to this lovely team of sweet professionals! I will be grateful to each one of you for the rest of my life!
